Certification Details

• American Heart Association certification is required for all direct patient care personnel
• Attach scanned copy of certification (with expiration date) to candidate's record in Trinity VMS
• Must include an image of the front and back of the BLS card
• Must be dated within 30 days of start for any position requiring a license or certification
• Attach scanned on-line verification page with a machine-generated date indication when the verification was performed within past 30 days
• Ensure uploading the state verification if available with NURSYS for RNs and LPNs (California ONLY - must also provide picture of HARD COPY license for RN)

Job Requirements

• Attach scanned proof of 2 vaccinations with dates or primary source IgG titer with lab print-out to candidate's record in Trinity VMS (MMR)
• Must have 1st vaccination in series prior to start; 2nd shot in series 30 days later; 3rd shot in series 6 months later (Hepatitis B)
• Attached scanned proof of 1 vaccination with date or primary source IgG titer with lab print-out to candidate's record in Trinity VMS (Varicella)
• All engaged contractors must have page 27 signed form on file in Trinity VMS (Code of Conduct)
• Standard i9 completed by the agency
• Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement must be signed and on file
• Signed Trinity Integrity and Audit Services Form within days prior to start

• Attach scanned EPLS report dated within past 30 days into candidate's record in Trinity VMS
• Attach scanned OIG result print-out dated within past 30 days into candidate's record in Trinity VM
• SCORE Exam within a year
• Must be dated within 30 days of start; check all names used and locations for NSO
• Must be completed and uploaded one week before start (Trinity System Access Form)

• Not required for non-clinical; agency will provide checklist for clinical contractors (Trinity Skills Checklist)
• Resume, Skills Checklist within a year, 2 Professional References required (Trinity Profile)
• Proof of COVID-19 vaccine Sept 2025 booster, declination or exemption with agency letterhead; declination not accepted for CA or IL; end date October 2026
• Proof of Influenza vaccine or declination with agency letterhead; declination not accepted for CA or IL; flu dates 10/1/2025-4/30/2026
• 10-panel drug screen completed within 30 days of start including methadone, phencyclidine, benzodiazepines, cocaine, propoxyphene, amphetamines, opiates, 6-acetyl morphine, methaqualone, barbiturates
