Job Requirements
- Must have completed Trinity Profile including Resume and Skills Checklist within a year
- Must complete Trinity Skills Checklist (provided by agency) for clinical contractors
- Must provide documentation of Personal Health History including Fit Test, 2 Step PPD or IGRA, COVID-19 Vaccine, MMR, Hepatitis B vaccination series, Varicella, Physical/Health Statement, Influenza Vaccine (optional), and Trinity Color Vision
- Must complete Trinity 10 Panel Drug Screen within 30 days of start
- Must sign Trinity Confidentiality of Patient and Proprietary Information and Information System Security Agreement
- Must complete Trinity Conflict Of Interest Disclosure Form
- Must pass Trinity Criminal Background Check within 30 days of start
- Must provide Trinity Education Verification
- Must provide Trinity EPLS report dated within past 30 days
- Must provide Trinity GSA/OIG report dated within past 30 days
- Must sign Trinity Integrity and Audit Services Form prior to start
- Must complete Trinity Joint Commission Testing (CORE Exam) within a year
- Must provide Trinity NSO report dated within 30 days of start
- Must complete Trinity System Access Form one week before start
- Must provide License/Certificate Verification dated within 30 days of start
- Must sign Trinity Code of Conduct Form
- Must complete Trinity I9 form
- Must complete Trinity Specialty Specific Competency Testing within a year
