Responsibilities
- Perform personal care needs per plan of care, including bathing, feeding, and dressing
- Provide patients and families with a positive communication chain and a friendly and professional environment
- Meet the safety needs of patients using equipment safely and properly
- Plan and prepare nutritious meals, including shopping, as assigned
- Take and record oral, rectal, and auxiliary temperatures, pulse, respiration, and blood pressure when ordered
- Provide proper care and observation of patient skin
- Assess and report on patient condition and significant changes to the Case Manager
- Assist in ambulation and exercise as instructed by the hospice nurse or therapist
- Provide normal range of motion and transfers/positioning as directed
Requirements
- Must hold an active and unencumbered Certified Nursing Assistant license in this state
- Must have at least one year of experience as a CNA
- Ability to travel and work in multiple locations throughout Worcester and Hampden Counties
