Unit Overview

• Acute care hospital setting
• Collaborative team environment including RN Case Managers and Social Work Case Managers
• Average case management ratio approximately 25:1
• EMR Systems: Meditech and Midas

Requirements

• Recent acute care hospital case management experience
• Experience with Meditech EMR
• Experience using Midas
• Strong discharge planning and care coordination background

Preferred Qualifications

• Complex discharge planning experience
• Ability to manage high-volume caseloads in a fast-paced environment
• Strong communication and interdisciplinary collaboration skills
