Responsibilities
Manage inpatient case management functions within an acute care hospital setting. Collaborate with healthcare teams to coordinate efficient patient care. Review medical records and authorize up to 40 hours of RTO. Ensure compliance with hospital policies and procedures. Support patients through discharge planning and resource coordination.
