Host Healthcare is seeking a travel nurse RN Case Manager, Acute Care Case Management for a travel nursing job in Eureka, California.
Responsibilities
- Manage patient care plans and coordinate services across various departments
- Assess patient needs and develop treatment plans

- Ensure compliance with healthcare regulations and standards
Requirements
- Bachelor's degree in Nursing (BSN)
- Current California RN license
- Minimum 1 year of acute care case management experience
- Experience with healthcare regulatory requirements
