About the unit
- 20 beds, all private, all monitored
- Nurse–Patient Ratios: 1:1, 1:2, 2:1 depending on patient acuity
- Common diagnoses/types of patients:
- Post open heart surgical patients (CABG, Valves, CABG/Valve, Dissections, AAA, Vascular procedures, Carotids)
- Post cardiovascular procedures (STEMI, Unstable angina, Unstable N-STEMI, Cardiogenic Shock, Cardiomyopathy)
- Targeted Temperature Management (Hypothermia), Transcutaneous pacing, Transvenous pacing
- Severe heart failure, TAVRs, cardiac-related events requiring ICU care
- Ventilated patients, overflow patients from other ICUs including COVID patients
- Soon to come: Post VAD placement, Post heart transplant patient
- Specialty intervention equipment:
- IABPs, Impellas, Swan Ganz, ECMO, External Pacemakers, CRRT
- Common lines: PICC, Midlines, CVC, Swans, Arterial Lines (radial and femoral)
Required certifications and experience
- BLS, ACLS preferred; CCRN a plus
- Minimum two years of CVICU experience required; preference for travelers with ICU assignments
- Must be proficient in all cardiovascular ICU components (excluding immediate post-op, though that is a bonus)
Skills and expectations
- Ability to work at a fast pace
- Interpretation of cardiac strips
- Strong assessment skills, critical thinking, communication, organization, teamwork, compassion
- Ability to follow protocols and orders as written
- Motivation, time management, positive attitude and behavior
- RNs are required to titrate drips (e.g., dopamine, dobutamine, levophed, epi, neo, vasopressin, cleviprex, propofol, fentanyl, versed, insulin, heparin, esmolol)
Staffing and support
- Charge nurses 24/7; charge nurse may take an assignment
- House supervisor 24/7
- Patient Care Techs (PCTs) with ratios ranging from 1:12 to 1:15 (sometimes higher depending on staffing)
- CV APNs/PAs and CCS APNs/PAs majority of the time; providers on call
- Respiratory therapists 24/7
- Equipment includes ventilators, CPAP, BiPAP, heated high flow, and nitric oxide
- RNs expected to troubleshoot respiratory equipment; monitor techs also present
- All patients connected to Philips monitoring; RNs read and interpret their own strips
Scheduling and requirements
- Shifts are 12 hours (7a–7p or 7p–7a); not self-scheduling for non AHU Mission Partners; no block scheduling
- Weekend requirements: every other weekend (days: Sat/Sun; nights: Fri/Sat)
- Holiday requirements: every other holiday
- Floating requirements: float to every level of care; travelers expected to take a full team
- Dress code: no specific scrub color; must be appropriate in appearance; scrubs not provided
