Transportation Specialist (Houma FQHC)

About the role

The Transportation Specialist provides safe and reliable transportation for patients to and from scheduled appointments while maintaining a professional and compassionate attitude.

Responsibilities

  • Communicate effectively with patients and staff
  • Reports vehicle maintenance needs
  • Protects confidential information
  • Fulfills related services for health center patients as appropriate with respect to their confidentiality and privacy
  • Absorb and unload patients in a safe manner
  • Able to respond properly to medical and psychiatric emergencies
  • Completes all required training at initial hire, then as required by Administration (e.g. Crisis Intervention, Mental Illness, Non-Violent Practices, Workplace Violence etc)
  • Assist clients in safely boarding and exiting the vehicle (e.g. wheelchair safety)
  • Maintains daily log of vehicle mileage
  • Maintains patient signed release for each patient transported
  • Maintains log of routine inspections of vehicle, daily, weekly and monthly
  • Ensures patients arrive in a timely manner for all clinic appointments
  • Communicates with clinic staff regarding emergencies, traffic, and patients unable to keep their scheduled appointment
  • Performs other related duties as requested

Requirements

  • High School diploma or GED
  • Over 18 years of age
  • Possess a valid driver’s license
  • Current CPR certification
  • Prior experience (preferred)
  • Knowledge of Patient Centered Medical Home Model (PCMH) and team based approach to care
  • Maintains awareness of potential risk to clients, staff and facilities and notifies administration as needed

  • Communicate effectively with clinic staff, patients and families
  • Consults with facility personnel and administrative field personnel, collects and analyzes data to identify needs for the assigned special services program within the particular service area
  • Maintains a clear driving record in order to be able to drive company vehicles when necessary for transportation needs of those we serve
  • Notifies administration of any mechanical issues or needs with regards to the vehicle
  • Maintains all receipts and data in a confidential manner
  • Attends weekly and/or monthly staff meetings
