About the role
The Transportation Specialist provides safe and reliable transportation for patients to and from scheduled appointments while maintaining a professional and compassionate attitude.
Responsibilities
- Communicate effectively with patients and staff
- Reports vehicle maintenance needs
- Protects confidential information
- Fulfills related services for health center patients as appropriate with respect to their confidentiality and privacy
- Absorb and unload patients in a safe manner
- Able to respond properly to medical and psychiatric emergencies
- Completes all required training at initial hire, then as required by Administration (e.g. Crisis Intervention, Mental Illness, Non-Violent Practices, Workplace Violence etc)
- Assist clients in safely boarding and exiting the vehicle (e.g. wheelchair safety)
- Maintains daily log of vehicle mileage
- Maintains patient signed release for each patient transported
- Maintains log of routine inspections of vehicle, daily, weekly and monthly
- Ensures patients arrive in a timely manner for all clinic appointments
- Communicates with clinic staff regarding emergencies, traffic, and patients unable to keep their scheduled appointment
- Performs other related duties as requested
Requirements
- High School diploma or GED
- Over 18 years of age
- Possess a valid driver’s license
- Current CPR certification
- Prior experience (preferred)
- Knowledge of Patient Centered Medical Home Model (PCMH) and team based approach to care
- Maintains awareness of potential risk to clients, staff and facilities and notifies administration as needed

- Conducts themselves in a manner of professionalism that is conducive to patient care in an organized and efficient manner
- Communicate effectively with clinic staff, patients and families
- Consults with facility personnel and administrative field personnel, collects and analyzes data to identify needs for the assigned special services program within the particular service area
- Maintains a clear driving record in order to be able to drive company vehicles when necessary for transportation needs of those we serve
- Notifies administration of any mechanical issues or needs with regards to the vehicle
- Maintains all receipts and data in a confidential manner
- Attends weekly and/or monthly staff meetings
