About the Role
Responsible for coordinating and providing transportation for residents to and from physician appointments in a timely manner under the supervision of licensed nursing personnel.
Responsibilities
- Organizes transportation of residents to complete required appointments based on priority of resident needs.
- Transports residents to and from inpatient and outpatient programs, facilities, and residences.
- Develops driving schedules based on residents’ scheduled medical appointments and program destinations.
- Provides transportation for current or new residents from hospitals after discharge.
- Reports and documents resident behavior, changes in condition, or abnormal findings (e.g., consciousness, pain, bleeding, vital signs) during transportation to clinical supervisors.
- Transports medical records for signatures to medical offices.
- Maintains van safety, cleanliness, and working condition; logs mileage and ensures routine maintenance is performed.
- Assists clinical staff with safe lifting, transferring, and transporting residents as specified by physician orders or clinical supervisors.
- Establishes positive communication with outside facilities, hospitals, physicians' offices, and specialty clinics to ensure proper resident services.
- Reports scheduling changes promptly to the Director of Nursing (DON), nursing staff, and ancillary service providers; follows up as necessary.
- Provides adequate hydration to residents at regular intervals based on individual needs.
- Protects the personal property of residents.
- Performs other duties, responsibilities, or activities as assigned or required.
Qualifications
- High School Diploma or equivalent (preferred).
- Valid Driver’s License and clean driving record.
- Current BLS/CPR certification (preferred).
