Job Summary and Responsibilities

As an RN Transplant Coordinator, you will be a pivotal force in transforming lives, orchestrating the intricate journey of transplant patients from referral to recovery.

• Organizes the clinical program
• Develops, implements, reviews, revises and maintains policies, procedures, and standards of care for patient population
• Develops, implements, and maintains ongoing continuous quality improvement activities for the clinical program.
• Serves as resource to hospital staff regarding patient care.
• Captures and manages patient evaluation process.
• Performs initial assessment of patient and family according to protocol for acceptance into program

Required

• Graduate of accredited school of nursing
• Three (3) years experience with patients in a related setting or three (3) years experience in an intensive care, acute care, telemetry or medical / surgical setting
• Registered Nurse: AZ

Preferred

• Bachelors of Nursing or higher degree
• Critical care experience
• Program Management/Development experience
• Care Coordination experience
