Transitions of Care Pharmacist - Full Time
About the role
The Transitions of Care Pharmacist plays a crucial role in optimizing medication therapy management to improve clinical, economic, and humanistic outcomes. They engage in activities such as comprehensive medication reviews, targeted interventions, medication reconciliation services, and discharge assistance.
Responsibilities
- Conducts Comprehensive Medication Reviews and Targeted Interventions to assess medication appropriateness and optimize therapy.
- Engages in patient-centered medication therapy management through transitional care, including medication reconciliation services and discharge assistance.
- Assists with managing medications and costs through coordination of benefits, prior authorization, formulary adherence, and avoiding unnecessary medication therapies.
- Collaborates with the outpatient pharmacy and consults with the prescriber, interdisciplinary care team, patient, and/or caregiver to develop patient-specific disease state therapy goals.
- Provides interdisciplinary support and collaboration, communicates and documents clinical recommendations/interventions to prescribers and the interdisciplinary care team.
- Coordinates medication reconciliation across the continuum of care and monitors to improve clinical outcomes and prevent readmissions, lapses in care, and adverse drug reactions.
- Assists with medication-related plans for the day of discharge and conducts follow-up with high-risk patients during the 30-day post-discharge period.
- Identifies additional therapies needed or therapies no longer needed prior to discharge.
- Aids in staffing as necessary by the Director of Pharmacy.
- Identifies the needs of the patient population served and modifies and delivers care specific to those needs.

Qualifications
- Bachelor's Degree in Pharmacy or PharmD from an ACPE accredited pharmacy school.
- Minimum of one year of work experience, preferably in an acute care setting.
- Strong clinical therapeutic skills and knowledge of appropriate medication dosing in geriatric and adult populations.
- Self-directed and possesses excellent organizational and communication skills.
- Excellent written and verbal communication skills.
- Proficient computer skills, including Google Suite and Microsoft Office platforms.
Education, Knowledge, Skills and Abilities Preferred:
- Two years of work experience.
- Pharmacy Residency (PGY-1).
- Performance of the 5 Core Elements of Medication Therapy Management Services: Comprehensive Medication Therapy Review (CMR), Personal Medication Record (PMR), Medication-Related Action Plan, Interventions and Referrals, Documentation of Services and Follow-Up.
- Communicates effectively and possesses strong verbal and written skills.
- Demonstrates excellent listening skills and maintains empathy with callers.
- Able to articulate information in a clear and concise manner and be able to relate to peers, prescribers, interdisciplinary care team, patients, and/or caregivers.
- Able to employ the teach back method during patient/family counseling.
- Able to probe and question patients and providers to extract information to understand concerns and needs.
- Able to maintain a professional demeanor on the phone and deal with healthcare providers, patients, and/or caregivers.
- Able to respond to providers and patients in a supportive manner.
- Able to defuse potential customer conflicts using appropriate tone, support, questions, and/or counseling.
- Able to adopt a customer service approach to problem-solving and operations.
- Self-motivated and works independently with minimal supervision.
- Able to appropriately treat private and confidential information as required by HIPAA.
- Adheres to company policies and procedures and federal and state rules and regulations.
- Able to demonstrate proficiency of computer applications. EPIC experience preferred.
Licenses and Certifications Required:
- Registered Pharmacist License.
Licenses and Certifications Preferred:
- Board Certified Pharmacotherapy Specialist.
- Medication Therapy Management Certification.
