About the role

The Facilitator embraces CII’s North Star and serves children, youth, and families through a whole-family approach, providing program participants with access to comprehensive services and support to reach their full potential.

Responsibilities

  • Uses independent judgment and discretion to conduct regular home visits to determine program participant needs and provides skills building, guidance and support to families.
  • Participates in and remains available to assist in crisis intervention and to provide support and assistance to program participants in resolving problems.
  • Uses independent judgment and discretion to provide case planning, linkage, and support services to children, youth, and families, including internal linkages to supportive services, referrals, and coordination with educational and employment services, pediatric and dental care, and other services as needed.
  • Ensures timely and quality documentation of participant services in accordance with all agency and contractual requirements, including ensuring records are always audit ready.
  • Represents CII at DCFS, DMH and community meetings; promotes CII’s goals and philosophies; participates in community committees and networks; and actively collaborates with others.
  • Establishes and maintains rapport and effective working relationships with children, youth, and families and works effectively within CII and community at large.
  • Uses independent judgment and discretion to assess and identify participant’s needs. In collaboration with participants, develops meaningful goals, monitors progress, measures program outcomes, and provides community referrals.
  • Tailors services and supports to be accessible, coordinated, appealing, non-stigmatizing, trauma-informed, and developmentally appropriate, building on strengths and protective capacities and enabling program participants to pursue their goals.
  • Advocates on behalf of the participant with other agencies, government programs and similar providers to help them receive needed services.
  • Participates in integrated case conferences and team meetings to monitor individual needs and services plans, ensuring effective quality clinical service delivery in alignment with CII’s whole-family approach to services.
  • Safeguards the confidentiality, integrity, and availability of patient protected health information (PHI).
  • Ensures accurate and timely completion of outcome and evaluation testing and reports required by the program.
  • Develops and maintains effective working relations with other agencies, schools, community organizations, placement centers and other resources to improve services for CII clients.

Qualifications

  • Master’s Degree in Social Work, Psychology, Marriage and Family Counseling, or a closely related field; Case Management experience preferred; Early Childhood Development experience preferred; Experience working with children, youth, and families involved in the child welfare system preferred.
  • Valid driver's license and state-required auto insurance; Required to travel to sites extensively (> 50% of the time).
  • Spanish / English bilingual preferred; Strong ability to work with others towards a common goal; Ability to handle confidential information.
  • Working knowledge of computer programs in a Windows environment, including Word, Excel and E-mail.
