Transitional Care Nurse - Home Health RN

Responsibilities

• Support patient self-management
• Advocate for patients and families
• Provide education and coaching
• Counsel patients and families
• Utilize the nursing process for ongoing assessment and outcome management
• Collaborate with the care team for patient-centered planning

Requirements

• Bachelor of Science in Nursing (BSN)
• Active Registered Nurse (RN) licenses in Missouri (MO) and Kansas (KS)
• At least 2 years of experience
• Experience in Home Health, Hospice, or Case Management preferred
• Active Basic Life Support (BLS) certification
• Clean driving record
