SPECIFIC RESPONSIBILITIES
- Run monthly workshops covering key life skill competencies as per ACS requirements.
- Ensure connections to community resources.
- Aid youths in completing housing applications (NYCHA, Emergency Housing Vouchers, etc.) as needed.
- Obtain and document all vital documents for youths on caseload, especially well before discharge.
- Provide supportive care for youths leaving placement before age 21, adhering to ACS regulations.
- Document all contacts in Connections within required timeframes.
- Complete PYA checklist using ACS web-based PAT tool.
- Attend team house meetings and conferences as necessary.
- Secure and place youths in internships based on skill level.
- Assist with annual SYEP placement.
- Collaborate with caseworkers and team members to help youths set and achieve goals.
- Maintain and submit monthly tracking grids on time.
- Timely reconcile monthly expenses.
- Travel to various locations in Queens and Brooklyn to support youths.
Qualifications
- Minimum of two years’ experience working with young adults.
- Experience with developmentally disabled youth and knowledge of Motivational Interviewing is a plus.
- Bachelor’s degree in human services preferred.
Special Skills
- Familiarity with the community to which assigned and knowledge of major culture groups within the neighborhoods served.
- Desirable qualifications include residence in the community, fluency in the languages spoken by major groups served, and previous work or connection with the developmentally disabled population.
Relationship With Others
- Externally: interacts with community service providers to obtain and monitor service delivery.
- Internally: interfaces with agency business support, maintenance services, and quality improvement staff, as well as other PYA staff.
Working Conditions
- Typically a 9am - 5pm position, but flexibility is needed to accommodate youths' schedules.
Scope of Responsibility
- Responsible for meeting the stated requirements of the position.
Organizational Structure
- Works within the structure of the PYA program as it relates to the overall agency organization.
