About the role
The Transition Specialist plays a crucial role in assisting youth transitioning out of foster care by providing comprehensive case management services, advocating for their needs, and ensuring compliance with regulatory guidelines.
Responsibilities
- Assess personal needs and secure resources for clients
- Advocate for clients with various human services and legal systems
- Provide crisis intervention and develop action plans for youth
- Create and maintain detailed Individual Service Plans
- Maintain accurate and timely file documentation
- Follow HHS guidelines and policies related to PAL TFSS services
- Participate in and lead PAL program events and activities
- Work evenings or weekends as needed
- Implement BCFS HHS safety protocols
- Obtain necessary authorizations and approvals
- Show knowledge of community resources and demonstrate confidentiality
- Record services and activities accurately and report any suspected abuse
- Maintain professional and ethical standards
- Attend required trainings and comply with dress code
- Travel as needed
Requirements
- Maintain relevant certifications (First Aid, CPR)
- Hold a valid Texas driver’s license with a clear record
- Pass a pre-employment drug screen and background check
- Produce proof of work eligibility
- Be bilingual (Spanish required)
