Transition and Financial Specialist - Bilingual

About the role

The Transition Specialist plays a crucial role in assisting youth transitioning out of foster care by providing comprehensive case management services, advocating for their needs, and ensuring compliance with regulatory guidelines.

Responsibilities

• Assess personal needs and secure resources for clients
• Advocate for clients with various human services and legal systems
• Provide crisis intervention and develop action plans for youth
• Create and maintain detailed Individual Service Plans
• Maintain accurate and timely file documentation
• Follow HHS guidelines and policies related to PAL TFSS services
• Participate in and lead PAL program events and activities
• Work evenings or weekends as needed
• Implement BCFS HHS safety protocols
• Obtain necessary authorizations and approvals
• Show knowledge of community resources and demonstrate confidentiality
• Record services and activities accurately and report any suspected abuse
• Maintain professional and ethical standards
• Attend required trainings and comply with dress code
• Travel as needed

Requirements

• Maintain relevant certifications (First Aid, CPR)
• Hold a valid Texas driver’s license with a clear record
• Pass a pre-employment drug screen and background check
• Produce proof of work eligibility
• Be bilingual (Spanish required)
