The San Diego LGBT Community Center (The Center) is a community non-profit established in 1972, dedicated to enhancing and sustaining the health and well-being of the lesbian, gay, bisexual, queer, transgender, nonbinary, immigrant, and HIV communities in the San Diego region. The Center is sex-positive, values-driven, and committed to equity, social justice, and client-centered, trauma-informed care.

About the role

Under the supervision of the Senior Director of Programs, this position focuses on meeting the individualized and critical needs of the transgender, nonbinary, agender, demi-gender, gender-fluid, two-spirit, genderqueer, intersex, gender nonconforming, questioning, and exploring community. The role uses peer-to-peer advocacy to assist clients in navigating gaps and barriers within various institutions and government agencies, ensuring culturally wraparound sensitive services to improve safety, stability, health, wellness, and psychosocial needs.

Responsibilities

  • Provide case management by creating individualized care plans to accomplish client goals, including navigating complex cases involving crisis, trauma, abuse, discrimination, self-harm, housing insecurity, systemic oppression, and exclusion.
  • Provide linkages and referrals to gender-affirming and trans-related care in collaboration with internal and external partners.
  • Navigate health insurance systems (state and federally funded) to ensure access to equitable and trans-affirming healthcare.
  • Educate clients about their rights and assist in filing grievances and appeals with insurance plans and the Department of Managed Health Care.
  • Follow up on medical referrals and prior authorizations for treatment while maintaining HIPAA compliance.
  • Assist clients in securing employment, financial security, and enrollment benefits, particularly for those experiencing housing insecurity, displacement, or immigration-related challenges.
  • Facilitate support groups and the Project TRANS Advisory Committee.
  • Submit monthly program impact stories.
  • Serve as an ambassador for Project TRANS through outreach, tabling, and visible presence at LGBTQ+ events hosted by The Center and partners.
  • Participate in cross-departmental collaborations, including providing on-site support to youth at both youth centers.
  • Assist in planning and executing Project TRANS events, such as Transgender Day of Remembrance, Transgender Day of Visibility, Trans & Enby Summit, Trans Pride Village, and Gender Affirming Clothing Drives.
  • Gather and maintain accurate documentation of clients' goals and needs in internal data systems.
  • Complete monthly narrative reports for The Center’s Quality Assurance team and external funders.
  • Assist in administrative duties, including responding to inquiries, creating programmatic calendars, and managing purchases for discussion groups.

Requirements

  • 1 year of experience providing case management services to transgender, nonbinary, and gender-expansive communities.
  • Fluency in English.
  • Proficiency in PC/Windows-based environments and Microsoft Office.
  • Demonstrated ability to manage multiple tasks with shifting priorities.
  • Competence in working with diverse populations.
  • Competence in using strength-based, trauma-informed, and motivational interviewing techniques.

Preferred Qualifications

  • Bachelor’s degree in a social service-related field.
  • Fluency in Spanish.
  • 2 or more years of experience providing case management services to transgender, nonbinary, and gender-expansive communities.
  • 1 year or more of experience providing case management services to people living with mental illness and/or substance use disorders.
  • Knowledgeable about current issues related to the LGBTQIA+ community.

Schedule

  • On-site, in the office, and in the field with program participants.
  • Occasional evening and Saturday work required for programming and events.
  • Office located in the Hillcrest neighborhood of San Diego.

Pay

The pay range for this position is $23.00 - $24.25 per hour, commensurate with experience.
