Transfer Nurse (LVN) - FT Nights | Voyages Conroe

About the role

This position is responsible for providing high quality, prompt assistance with triaging and coordination of transfers from other facilities. The transfer center nurse monitors and responds to triage needs, completes the direct referral acceptance process, and uses technology to review patients' clinical needs. The nurse delivers prompt communication of acceptance or deflection and collaborates closely with referral sources, physicians, nurses, and others. This is an exempt position.

Responsibilities

• Triage and coordinate transfers from other facilities
• Monitor and respond to triage needs
• Complete direct referral acceptance process
• Use technology to review patients' clinical needs
• Deliver prompt communication of acceptance or deflection
• Collaborate with referral sources, physicians, nurses, and other stakeholders

Requirements

• Valid LVN license
• Completion of Crisis Prevention Training (CPI) and CPR within 30 days of hire and prior to patient contact
• Previous experience in behavioral health and substance use field
• Experience working with case management, hospitals, and admissions preferred
• Excellent customer service, verbal, and written communication skills
• Strong organizational and multitasking skills
• May be required to work flexible hours
• Position may be facility-based or remote

Schedule

Shifts will vary to support this 24/7 department and may include day and overnight coverage with shift times that vary over different US time zones.
