Responsibilities
- Anticipates and responds to all Transfer Center calls.
- Captures and communicates with referring facilities regarding transfer and bed assignment.
- Develops and promotes long-term relationships with referring facilities and providers.
- Maintains and monitors quality of care and services provided.
- Evaluates and improves institutional capacity to assess impact on patient flow, resources, and capacity management.
- Supports the implementation of strategic goals related to capacity management, patient flow, and service-line development.
- Interprets and supports policies, procedures, and safety standards to facilitate daily operations.
- Performs other duties as assigned.
Qualifications
- Education: Bachelor’s Degree in Nursing or enrolled into a program within 6 months.
- Certification, Registration & Licensure: Current and active New Hampshire Registered Nurse license. American Heart Association Healthcare Provider BLS required on hire.
- Experience: Minimum five years of Acute Care Nursing experience. Previous supervisory/leadership experience desired.
- Skills: Demonstrated leadership ability and management skill, strong organizational skills, excellent interpersonal communication skills, and knowledge of Operational Excellence Guiding Principles.
