Therapy Manager

About the role

Lead, mentor, and transform patient care at home as a Therapy Manager for VNA Health Group. This full-time leadership role combines clinical oversight with team development and program growth to deliver exceptional rehabilitation services across Essex and Hudson Counties.

Responsibilities

• Provide clinical oversight and leadership for rehabilitation services across the service area.
• Guide, mentor, and support Physical Therapists, Occupational Therapists, and Speech-Language Pathologists.
• Conduct therapy evaluations and home visits as needed to support patient care and clinical excellence.
• Develop, implement, and oversee individualized plans of care that promote optimal patient outcomes.
• Manage and enhance rehabilitation programs to ensure effective service delivery and utilization.
• Monitor and analyze quality metrics, identifying opportunities for program improvement and growth.
• Serve as a clinical resource and educator for rehabilitation staff, fostering professional development and best practices.
• Collaborate with interdisciplinary teams to ensure coordinated, patient-centered care.
• Promote compliance with agency policies, regulatory requirements, infection control standards, and safety practices.
• Participate in Quality Assessment and Performance Improvement (QAPI) initiatives and organizational education programs.
• Support operational goals while cultivating a culture of accountability, engagement, and excellence.

Required Qualifications

• Bachelor's degree in Physical Therapy, Occupational Therapy, or Speech-Language Pathology.
• Current New Jersey professional license in your respective discipline.
• Minimum of 5 years of clinical rehabilitation experience.
• Strong clinical judgment and patient care skills.
• Exceptional communication, leadership, and interpersonal abilities.
• Proficiency with electronic documentation systems and technology.
• Valid driver's license and reliable transportation for travel throughout Essex and Hudson Counties.

Preferred Qualifications

• Master's degree in a related rehabilitation discipline.
• Previous supervisory, management, or team leadership experience.
• Experience in home health, community-based rehabilitation, or post-acute care settings.

Working Environment

This role combines field-based patient care, team leadership, and administrative responsibilities. Regular travel throughout Essex and Hudson Counties is required. Responsibilities include driving, sitting, standing, walking, lifting, and utilizing mobile technology for clinical documentation and communication.

Pay

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $92,000 to USD $120,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.
