About the role

As a Speech Therapist, you will primarily provide outpatient rehabilitation services in an in-home setting for the older adult population. This includes private residences, assisted living facilities, and independent living facilities. Powerback Rehabilitation To You allows you to evaluate and treat patients in their most functional environment—their home. You will organize and provide speech-language services to facilitate rehabilitation, directing patient participation in tasks to restore, reinforce, and enhance performance. This role promotes skills and functions essential for communication and swallowing, while offering a flexible schedule and work environment across various settings.

Responsibilities

  • Supervise CFYs, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
  • Adhere to department and patient treatment schedules, including timely clinical and billing documentation and work-related meetings.
  • Attend and contribute to patient care, staffing conferences, and other related meetings.
  • Make referrals to other disciplines or agencies to address comprehensive patient needs.
  • Consult with rehabilitation team members and physicians to select appropriate therapeutic regimens based on patient needs and capabilities.
  • Adapt programs and care plans according to individual patient needs.
  • Promote a safe environment and enforce fire and safety hazard elimination.
  • Order, clean, maintain, and report the need for repair of supplies and equipment.
  • Prioritize patient service, ensuring residents and families receive high-quality, compassionate care that recognizes individual needs and rights.
  • Screen, examine, and evaluate patients, including history, systems review, and application of tests and measures to establish diagnoses, identify impairments, and determine treatment plans.
  • Develop and implement treatment goals and methods in collaboration with patients and caregivers, including therapeutic techniques in areas such as:
    • Cognitive-communication
    • Speech intelligibility
    • Oral motor skills
    • Swallowing disorders/dysphagia
    • Perceptual abilities
    • Orientation and memory
    • Pragmatics and psychosocial expression
    • Functional communication
    • Case management in home and community settings
  • Coordinate the acquisition of adaptive communication devices and instruct patients in their use.
  • Identify hearing deficits and refer patients for assessment, diagnosis, rehabilitation, or personal amplification.
  • Select appropriate therapeutic activities based on the patient's physical and mental capacity, intelligence level, and interests.
  • Instruct patients in skills and techniques required for therapeutic activities and evaluate progress, attitudes, and behavior.
  • Perform routine reexaminations to modify or progress the treatment plan as needed.
  • Coordinate care in home and community settings with interdisciplinary team members and referring physicians.
  • Perform other related duties as required.

Requirements

  • Valid Speech Language Pathologist license in the state(s) of practice or proof of license eligibility.
  • Valid driver’s license or reliable, independent transportation to and from patients' residences.

Pay

USD $48.00 - USD $61.00 per hour.
