Role Overview

The Therapy Lead provides discipline-specific clinical support for therapy services (PT/OT/ST) in home health, ensuring safe, compliant, and effective care through mentorship, practice standardization, interdisciplinary collaboration, and quality improvement activities.

Responsibilities

• Evaluate patient function, complete assessments, review and update care plans, guide therapeutic activities, and document discharge planning.
• Collaborate with physicians and care teams, perform assessments like ROM, gait analysis, and measurements, and ensure regulatory and policy adherence.
• Serve as a clinical resource for complex cases, promote consistent therapy practices, review documentation, identify clinical risks, and support compliance with standards and regulations.

Qualifications

• Master’s Degree in PT, OT, or SLP, with licensure in the respective field.
• At least 3+ years of home health experience.
• Demonstrates clinical expertise and ability to guide peers.
