Therapist-Physical I PRN

Responsibilities

• Evaluates and assesses the patient based on subjective and objective information.
• Develops a written plan of care.
• Provides treatment according to established goals on the plan of care.
• Evaluates effectiveness of the plan of care and reassesses patient's functional status as needed.
• Provides discharge planning information which includes patient/family education, follow-up, and referral to community resources.
• Contributes to the development of staff and students.
• Participates in activities designed to improve healthcare delivery.
• Documents interventions/care in the patient's medical record.
• Completes assigned goals.

Specifications

• Experience: Minimum Required 3 years clinical experience or advanced training and certification in area of primary assignment.
• Education: Preferred/Desired Minimum Required Training: Preferred/Desired Advanced training and/or certification in area of primary practice.
• Special Skills: Minimum Required Basic computer literacy skills.
• Licensure: Minimum Required BLS CERTIFICATION WITHIN 14 DAYS OF HER;PT.
