Key Responsibilities
- Conduct comprehensive telehealth assessments for newly enrolled Medicare clients
- Synthesize complex medical histories and identify clinical priorities for ongoing care coordination
- Document clinical findings that inform care manager (Hero) intervention strategies
- Provide clinical oversight and collaboration as needed with care management team
- Establish clinical credibility and trust during initial patient contact

Requirements
- MD or DO (must be licensed to practice in all 50 states)
- Board certification in family medicine, internal medicine or other relevant speciality
- Substantial experience in geriatric medicine, primary care, or chronic disease management
- Confident in telehealth assessment and skilled at building clinical rapport across a screen
- Strong clinical judgment and comfort with complexity and ambiguity
