Telehealth Preventative Primary Care Advanced Practice Provider- NP/PA

Responsibilities

• Conduct primarily telehealth video visits with occasional in-home visits
• Perform comprehensive assessments with a focus on prevention and chronic condition support
• Identify care gaps and coordinate care with team members
• Utilize technology and AI workflows to enhance member interaction time
• Communicate empathetically to foster trust with members and caregivers

Qualifications

• Active Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license required
• 3+ years of outpatient clinical practice in primary care, family medicine, internal medicine, or geriatrics
• Must obtain additional licenses within the first 90 days with support provided
• Active BLS certification at time of hire
• Comfortable delivering care via video telehealth and using electronic documentation systems

$115,000 – $140,000 annually
