Team Physician Associate Medical Director

Responsibilities

• Partner with the interdisciplinary care team and lead Interdisciplinary Group (IDG) meetings.
• Ensure a care plan is in place and anticipate patient needs while meeting regulatory requirements.
• Participate in in-person and virtual visits to document patient interactions and ensure access to benefits.
• Engage in a Call Coverage Rotation to address unanticipated needs and support newly admitted patients.
• Lead learning initiatives with the Interdisciplinary Team and the community on end-of-life issues.

Qualifications

• Maintain an active License and DEA in the state where care is being provided.
