Team Manager Home Care

About the role

The Team Manager leads the Patient Care Team, overseeing all patient care services, ensuring continuity of care, serving as a patient advocate, and coordinating with other healthcare providers.

Responsibilities

• Supervise, evaluate, and coordinate the interdisciplinary team
• Evaluate and approve patient admissions and discharges
• Serve as a patient advocate and coordinator for social services and healthcare providers
• Maintain and document patient records accurately
• Ensure timely recertification decisions
• Assure effective discharge planning
• Manage team schedule and services
• Facilitate interdisciplinary meetings
• Participate in on-call rotation
• Interview, select, train, supervise, and dismiss team staff
• Evaluate staff competence and performance
• Oversee staff and volunteer schedules
• Review and approve payroll
• Monitor clinical records and team activities
• Develop and implement performance improvement activities
• Ensure completion of patient/family satisfaction surveys
• Participate in surveys and regulatory requests
• Perform utilization reviews
• Address customer service issues and grievances
• Provide in-services to customers
• Verify and approve patient care services
• Control and manage costs related to patient care services
• Attend inservices, educational seminars, and workshops
• Develop and achieve professional growth goals
• Mentor newly hired Team Managers

Requirements

• Reliable transportation with appropriate license and insurance coverage
• Private telephone in home
• Knowledge of primary medical and nursing care principles
• Understanding of interdisciplinary health care team dynamics
• Working knowledge of VITAS information system
• Two years supervisory experience or equivalent
• Bachelor's degree preferred
• Current and valid R.N. License
