About the role
The Team Manager leads the Patient Care Team, overseeing all patient care services, ensuring continuity of care, serving as a patient advocate, and coordinating with other healthcare providers.
Responsibilities
- Supervise, evaluate, and coordinate the interdisciplinary team
- Evaluate and approve patient admissions and discharges
- Serve as a patient advocate and coordinator for social services and healthcare providers
- Maintain and document patient records accurately
- Ensure timely recertification decisions
- Assure effective discharge planning
- Manage team schedule and services
- Facilitate interdisciplinary meetings
- Participate in on-call rotation
- Interview, select, train, supervise, and dismiss team staff
- Evaluate staff competence and performance
- Oversee staff and volunteer schedules
- Review and approve payroll
- Monitor clinical records and team activities
- Develop and implement performance improvement activities
- Ensure completion of patient/family satisfaction surveys
- Participate in surveys and regulatory requests
- Perform utilization reviews
- Address customer service issues and grievances
- Provide in-services to customers
- Verify and approve patient care services
- Control and manage costs related to patient care services
- Attend inservices, educational seminars, and workshops
- Develop and achieve professional growth goals
- Mentor newly hired Team Managers
Requirements
- Reliable transportation with appropriate license and insurance coverage
- Private telephone in home
- Knowledge of primary medical and nursing care principles
- Understanding of interdisciplinary health care team dynamics
- Working knowledge of VITAS information system
- Two years supervisory experience or equivalent
- Bachelor's degree preferred
- Current and valid R.N. License
