Team Director

About the Role

The Team Director at the ACT program leads a mobile, multi-disciplinary team providing integrated, evidence-based treatment, rehabilitation, case-management, and support services to individuals diagnosed with severe mental illness. The role is split evenly between direct patient services in the community and administrative/supervisory duties, ensuring high-quality, person-centered care and smooth program operations.

Responsibilities

• Dedicate 50% of time to direct patient services in the community and 50% to administrative/supervisory tasks.
• Oversee day-to-day clinical and operational functions of the ACT team, ensuring compliance with contractual requirements and adequate coverage for client needs.
• Use outcomes data and client feedback to drive service quality improvements.
• Assist the Program Director with hiring, orienting, and evaluating staff; provide training through supervision and team meetings; address performance issues.
• Ensure timely adherence to all reporting requirements, including staff documentation and system entries.
• Conduct side-by-side contacts with staff as part of direct patient services.
• Provide regular supervision to clinical staff via daily team meetings, case conferences, and one-on-one sessions.
• Lead daily team meetings and review meeting minutes.
• Carry a caseload equivalent to half of a full caseload.
• Actively participate in the intake process for new referrals and review clients’ ongoing service plans.
• Supervise program interns directly or oversee their clinical supervision by licensed team members.
• Run specialized groups or provide clinical coverage in the absence of team members.
• Serve as a key resource for community outreach, establishing linkages with shelters and relevant organizations.
• Complete all required documentation, including progress notes, risk assessments, and other paperwork.
• Provide after-hours emergency coverage via cell phone on a rotating basis.

Requirements

• Master’s Degree or higher in social work, psychology, rehabilitation counseling, or a related field.
• New York State licensure as one of the following: LMSW, LCSW, LMHC, LMFT, Licensed Psychologist, or Licensed Creative Arts Therapist.
• Minimum of three years of related work experience.
• Minimum of one year of direct clinical supervision preferred.
• Specialized experience with substance use disorders, vocational services, family therapy, and/or homelessness is a plus.
• Previous work on an ACT team or Mobile Crisis team is a plus.
• Proficiency in Microsoft Office and electronic medical records/data entry.
• Ability to read paper and electronic documents and perform significant data entry.

Work Environment

50% of the work is performed in the community (e.g., shelters, supportive housing, or client residences), and 50% is on-site at the program location.
