Tampa, Florida - PA or NP for Endocrinology Practice

About the role

Elaya Health is a rapidly growing endocrinology practice specializing in providing high-quality care to patients in skilled nursing and long-term care facilities. We are seeking an experienced and compassionate Nurse Practitioner or Physician Assistant to join our expanding team.

Responsibilities

• Perform on-site patient evaluations and follow-up visits
• Manage patients with diabetes, thyroid disease, and other common endocrine conditions
• Review blood glucose trends, laboratory results, and medication regimens
• Optimize diabetes management and help reduce unnecessary finger sticks and medication burden
• Collaborate with nursing staff, DONs, facility leadership, and primary care teams
• Provide clear clinical recommendations and ongoing follow-up
• Document in facility EMRs, with training and administrative support provided
• Help reduce avoidable hospitalizations through proactive specialty care

Qualifications

• Licensed Nurse Practitioner or Physician Assistant in New York and/or New Jersey, or willingness to obtain licensure
• Experience in endocrinology, internal medicine, geriatrics, or skilled nursing preferred
• Endocrinology experience is a plus but not required for the right candidate
• Strong clinical judgment and communication skills
• Comfortable working independently while collaborating with a larger clinical team
• Patient-centered and committed to providing high-quality care to the long-term care population
