About the role
Serving six counties in Northern Michigan, North Country Community Mental Health is a public behavioral health organization committed to improving the quality of life for people with mental illness and/or developmental disabilities in our communities.
Responsibilities
- Develop, review, deliver and modify as needed, a Person-Centered Plan of Service in collaboration with the client, parent or guardian, designated advocate, residential and day services support workers, and/or clinical professionals.
- Act as a link between the service provider system including agency staff; the client’s primary care physician, and other community health care and human services providers; and the client, and /or parent or guardian.
- Assist in the development of natural supports.
- Monitor the quality of services, and the health and safety of the client.
- Assure that services and supports authorized in the plan of service are in place and are occurring.
- Consult with nurse and/or physician on significant new, or changes in, clients’ physical healthcare needs.
- Cookordinate resources and provide assistance during crisis.
- Cookordinate residential and financial supports planning to help determine type and scope of supports available for residential suitability.
- Assist in attaining and maintaining benefits, such as Medicaid, SSI/SSD, food stamps, etc.
- Provide information regarding rights, alternatives to guardianship, and guardianship proceedings and paperwork.
- Provide complete, timely, and accurate documentation in the clinical record in accordance with agency policy and procedure, and Medicaid requirements.
- Monitor and ensure that applicable laws, rules, policies, and procedures are followed.
- Assist in reporting, processing, or investigating cases of alleged abuse or neglect.
- Work with agency Recipient Rights Officer and with Michigan Department of Health and Human Services Protective Services Workers or Licensing Consultants as needed.
- Assist in transporting and accompanying clients to medical appointments, evaluations, as needed.
- May be required to participate in 24-hour emergency services on-call system to conduct or coordinate emergency visits to homes or hospitals, as needed.
- May assist, arrange, or provide in-service training programs for clients, direct support workers, foster care providers, parents, guardians, and interested community members or civic organizations.
- May provide supervision to supports coordinator assistant when applicable.
Qualifications
- Education: Minimum of a bachelor’s degree in social work or other relevant human service required, with licensure/limited licensure as appropriate from State of Michigan; Must be either a Qualified Intellectual Disability Professional (QIDP) or Qualified Mental Health Professional (QMHP).
- Qualified Intellectual Disability Professional (QIDP): Individual with specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons with intellectual or developmental disabilities as part of that experience) OR one year of experience in treating or working with a person who has intellectual disability.
- Qualified Mental Health Professional (QMHP): Individual with specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience) OR one year of experience in treating or working with a person who has mental illness.
- Experience: One year of working with the IDD or SPMI population. Able to meet requirements as a Qualified Intellectual Disability Professional (QIDP) or Qualified Mental Health Professional (QMHP).
- Proficient computer skills including Office 365.
- Other: Ability to interact with physicians and other healthcare providers in a professional manner. Working knowledge of physical and mental health, chronic health conditions, and behavioral health medications.
