Supports Coordinator (26-100) HYBRID

About the role

This role is responsible for Person-Centered planning to develop an Individual Plan of Service. The Supports Coordinator will provide on-going linking, coordinating and monitoring of services.

Responsibilities

• Provide on-going linking, coordinating and monitoring of services
• Develop and implement Person-Centered planning
• Meet with clients in the Kent County community as part of their responsibilities

Requirements

• Bachelor’s degree in human services field AND one year of experience treating or working with a person who has intellectual disability or specialized training received through fieldwork and/or internships
• Possession of a valid driver’s license and access to reliable transportation for job related use
• Strong organizational skills, with experience in person-centered planning
• Knowledge of Culture of Gentleness
• Demonstrated understanding of persons with developmental disabilities
• Demonstrated communications skills that effectively convey information
• Demonstrated active listening, i.e. listening to what other people say and asking appropriate questions
• Demonstrated time management skills
• Lived experiences with mental illness/developmental disabilities/substance use disorders valued

Qualifications

• Minimum qualifications: Bachelor’s degree in human services field AND one year of experience treating or working with a person who has intellectual disability or specialized training received through fieldwork and/or internships
• Preferred qualifications: Licensed BSW Knowledge of trauma-informed theories, principles, and practices

Schedule

This is a full-time, hybrid position, working Monday to Friday from 8:00am to 5:00pm. While the position is remote, the employee in this role will meet with clients in the Kent County community as part of their responsibilities.
