Responsibilities

• Provide person-centered care to individuals with diverse needs in home and community settings
• Assist with daily living activities, personal care, medication reminders
• Support community outings to promote independence, safety, and dignity
• Collaborate closely with healthcare teams and families
• Participate in ongoing training to enhance skills in behavioral support and rehabilitation
