34 hours per week, days and nights.

Responsibilities

  • Carry out needs and risk assessments, conduct support and care plan reviews, and provide care and support to all customers accepted onto the Self Enablement Service.
  • Engage with customers on an agreed basis to support individuals to achieve and maintain independent living.
  • Provide information, advice, and advocacy on welfare rights, housing options, tenancy rights and responsibilities, focusing on helping customers develop skills and knowledge in these areas.
  • Provide information and assistance to customers to enable them to make choices about developing their social network and participating in their local community, including employment, education, and training opportunities.
  • Work with customers to identify support needs that can be met by other agencies, make referrals to specialist agencies, and develop appropriate casework links with such agencies in relation to customers' support plans.
  • Provide care and support in accordance with current best practice, policy and procedures, agreed standards, legislative requirements, and relevant regulations under the direction of the Registered Manager.
  • Deliver a high-quality service, ensuring outstanding customer service and service delivery by acting as an ambassador for Humbercare and as a point of contact for customers.
  • Engage with customers to design services that work for them and utilize local resources to make a real, quality difference in people's lives.

Requirements

Essential

  • Educated to Level 2 Diploma or equivalent.
  • Experience of working in a social care or similar environment.
  • Knowledge of the Care Quality Commission and the Key Lines of Enquiry and fundamental standards.
  • Knowledge of varying complex needs and their impact on the ability for a person to access housing or maintain independent living.
  • Knowledge of local statutory and voluntary support agencies.
  • Working knowledge of person-centred, strengths-based approaches to support and care planning.
  • Working experience of dealing with complex service users/customers/public.
  • Experience of working with others to achieve successful outcomes.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Team working experience, working cooperatively with colleagues in complex, pressurized situations.
  • Excellent organizational and time management skills and the ability to effectively manage a caseload.

Desirable

  • Willingness to work towards a recognised housing/care/support qualification.
