Responsibilities

• Provide person-centered support to individuals with diverse abilities in home and community settings
• Guide and mentor staff to ensure quality care
• Implement and monitor individualized care plans

• Coordinate daily activities and monitor client progress
• Maintain accurate documentation and collaborate with families and clinical teams
