SCOPE OF ROLE
Reporting to the Director, RN Operations Addiction Medicine, the Supervisor, Community Nursing provides oversight, direction, and leadership of the community nursing team while ensuring high-quality and patient-centered care is received by patients and their families. The Supervisor, Community Nursing champions the implementation of new care pathways and protocols while leading service line team members. While primarily a supervisory role, provides direct patient care from time to time. The Supervisor, Community Nursing drives initiatives that contribute to patient access, engagement, and positive clinical outcomes.
HOW YOU’LL SUCCEED
- Effectively supervise and oversee the daily objectives of the Community Nursing team including, but not limited to, ensuring timely access to services, comprehensive assessment, individual census reconciliation and tracking, staff productivity, efficient and organized team communication, and thorough documentation.
- Effectively support cross-department collaboration and coordination of community-based patients and facilitate positive relationships with external partners.
- Oversee team members’ individual performance including holding regular face-to-face meetings, providing ongoing coaching, fostering development, and providing timely feedback as well as feedback through the annual performance review process.
- Plan, execute, and maintain daily nursing assignments and activities, facilitate nursing assessment of suicide risk, support patient medication access and adherence, and effectively oversee the nursing/provider handoff process.
- Provide ongoing communication, teaching, and training for all team members.
- Actively lead team meetings and process improvement initiatives to continuously improve work productivity, quality, and efficiency of the team.
- Support the Nursing Director in developing and implementing evidence-based clinical protocols, strategies for care coordination, and metrics for monitoring program effectiveness.
- Ensure clinical activities and procedures are compliant with state and federal standards, as well as evidence-based practices.
- Participate in recruitment and retention activities.
- Assist team with patient care as needed, modeling and driving clinical excellence. Plan, execute and maintain daily productive nursing assignments/prescriber pairings to meet patient care needs.
- Participates in after-hours call to accept potential admissions.
KNOWLEDGE & EXPERIENCE
- Bachelor Degree in Nursing preferred.
- Valid unencumbered RN license in the state of Ohio required.
- 3+ years of experience in an ambulatory, acute, residential or inpatient psychiatry preferred.
- 2+ years of nursing supervisory experience in healthcare preferred.
- American Heart Association (AHA) Basic Life Support (BLS) certification required.
- Excellent written and verbal communication skills
- Proficiency in Microsoft Office products
WORKING CONDITIONS
- Work is normally performed in a typical interior/office/clinical work environment/community/ patient home
- While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.
- Requires periods of sitting, standing, telephone, and computer work.
- Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.
- Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.
- Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.
- Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.
- Physical effort required: frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.
- Sufficient dexterity to operate a PC and other office equipment.
