Supervisor for Community Health Workers

About the Role

This is a grant-funded, full-time position with day shift hours. The role provides supervision, coaching, and direct support to a team of Community Health Workers (CHWs) working to improve health outcomes and eliminate racial, ethnic, and economic disparities. The position plays an integral role in program management, fostering CHW growth, ensuring quality service delivery, and guiding progress towards intervention goals.

Responsibilities

  • Daily operations and oversight of CHWs, including hiring, training, professional development, performance evaluations, and onboarding/offboarding
  • Coordinate staff scheduling and assignments; review and approve administrative functions (time, payroll, expenses)
  • Collaborate with department leadership to monitor program performance, quality improvement, and service delivery
  • Align programs, workflows, and referral processes internally
  • Provide leadership and guidance on best practice techniques to CHW staff
  • Implement departmental plans and priorities identified by accountable leaders
  • Assist with the design and evaluation of CHW interventions, including integration into clinical care
  • Promote CHW services within the health ministry and community through events, speaking engagements, meetings, in-services, and distribution of program materials
  • Effectively communicate goals, standards, program expectations, service performance, and how the work serves Trinity Health objectives

Qualifications

  • High school diploma or equivalent
  • Five (5) years of experience as a Community Health Worker (paid and non-paid experience/internships accepted)
  • Education can be considered in lieu of required work experience
  • Previous experience managing people, programs, or projects
  • Must complete CHW Supervisor training within 6 months of employment
