This Supervisor will oversee a team of Care Coordinators and Engagement Coordinators who provide Care Coordination outreach and services to our unengaged member population.
Responsibilities
Medica’s Supervisor is responsible for managing teams of care coordinators and engagement coordinators for the provision of care coordination services for State Public Program and other Medica members. The majority of members will be from our Care Coordination Products (MSHO, MSC+, SNBC) within State Public Programs and the Government division. In addition, this leader will partner with our Clinical Care Manager in the delivery and evaluation of Medica Care System care coordination services, implementing improvement opportunities, managing essential care coordination duties and oversight of regulatory and quality compliance requirements.
Qualifications
- RN Diploma or Degree in Nursing (Associates or Bachelors) + a minimum of 2 years’ experience in community based services
- OR Bachelor's degree in Social Work + a minimum
