Job Summary
Under the general supervision of the Case Management Director, the supervisor of case management serves as a resource for the case management team and other departments. Responsible for providing direct supervision of the case managers.
Essential Functions
- Conducts assessments and documents conditions, observations, and actions.
- Supports case management coordination of patient transfers to acute care facilities to ensure compliance with hospital policies/procedures and regulatory requirements.
- Aid in managing the department in a cost-effective manner by utilizing current materials management techniques, maximizing human resources, and continuously reevaluating the manner in which services are delivered.
- Cover assignments of case management staff to meet required services in each area.
- Support case management coordination of patient transfers to ensure compliance with hospital policies/procedures and regulatory requirements.
- Directly provide case management to patients.
- Facilitate patient and/or family counseling sessions; establish goals and care plans.
- Participate as a contributing member in the discharge planning process.
- Serve as a liaison between hospital departments, including discharge planning and social service.
Knowledge/Skills/Abilities/Experience
- Requires critical thinking skills, decisive judgment, and the ability to work with minimal supervision.
- Must be able to work in a stressful environment and take appropriate action.
Qualifications
- Education: Associate Degree in Nursing (ADN) Required, Bachelor's degree Preferred.
- Licenses/Certifications: Professional Registered Nursing licensure in state of GA. Basic Life Support (BLS).
- Experience: Three (3) years of acute care experience, Previous Case Management experience.
