SUPERVISING COMMUNITY HEALTH WORKER/ EMERGENCY APPOINTMENTS HOMELESSNESS

This opportunity is part of an emergency recruitment to address the homelessness crisis declared by the LA County Board of Supervisors. The Department of Mental Health is seeking qualified candidates for Supervising Community Health Worker vacancies. Under the emergency order, applicants who meet the requirements may be hired for an initial period of up to 90 days, with an opportunity for permanent County employment based on a performance assessment.

About the Role

The Department of Mental Health is responding to the homelessness crisis by expanding services in housing, mental health, and substance use. This role involves supervising a team of Community Health Workers to provide direct health, mental health, and social services to clients and patients.

Responsibilities

• Supervise a team of Senior Community Health Workers and Community Health Workers, providing direction, monitoring schedules, and evaluating work quality.
• Assign service referrals from social workers, eligibility workers, and medical case workers to assist clients in obtaining food stamps, housing, medical services, mental health services, substance use care, and other supportive services.
• Discuss challenging cases with staff and recommend optimal courses of action for client/patient care.
• Lead and facilitate the development of independent living skills for clients, including support groups for housekeeping, cooking, budgeting, public transportation, and personal hygiene.
• Recommend and oversee improvements to work methods, policies, and procedures.
• Identify gaps in community programs and services and propose creative solutions.
• Advocate for client/patient access to County and community resources.
• Assist in planning, developing, implementing, and evaluating community health programs and supportive services in compliance with laws and regulations.
• Identify staff training needs and provide or coordinate training and coaching.
• Review staff documentation to ensure compliance with program and departmental policies and provide feedback for improvement.
• Serve as a program representative to internal and external partners.
• Supervise supportive services such as tutoring and parenting classes to facilitate client recovery and resilience.
• Interview prospective personnel and make employment recommendations.
• Carry a caseload of complex client/patient assignments as needed.

Requirements

• Training and Experience:
  • Option I: Three years of experience as a Senior Community Health Worker or similar role at a public or private community-based organization, clinic, hospital, or health system.
  • Option II: Five years of experience as a Community Health Worker at a public or private community-based organization, clinic, hospital, public health organization, or health system.
• License: A valid California Class C Driver License or the ability to use alternative transportation for job-related essential functions.
• Physical Requirements: Light physical effort, including occasional light lifting (up to 10 pounds) and some bending, stooping, or squatting. Considerable ambulation may be involved.

Definitions:

• Senior Community Health Worker: Assists in planning, developing, implementing, and evaluating community-based programs for clients with physical health, mental health, and social adjustment issues.
• Community Health Worker: Assists professional staff in providing direct health, mental health, or social services to clients and patients.

Additional Information

Under the emergency order, applicants who meet the requirements may be hired for an initial 90-day period. Performance will be assessed before the end of this period, with successful candidates considered for permanent appointment.
