Essential Duties And Responsibilities

  • Provides career navigation, coaching, and stabilization resources to facilitate positive outcomes for participants seeking to advance in career opportunities.
  • Delivers holistic and comprehensive strength-based case management.
  • Completes a comprehensive strength discovery assessment that provides quality information about each participant’s strengths, family supports, academic challenges, risks, and barriers to success, covering multiple domains, e.g., individual, family, school, and community.
  • Supports program participants in enhancing their successes through services such as community referrals, sending employment opportunities, and job retention coaching.
  • Conducts individualized goal development, long-term plans, and progress monitoring.
  • Meets with each participant weekly or bi-weekly based on needs identified in the assessment.
  • Mets with parents/caregivers to establish rapport and then, as needed, to provide support and connection to services.
  • Be available evenings and weekends to meet with participants and families.
  • Supports participants in creating clearly stated academic, training, and personal goals that address risk factors identified during the assessment process, which may include review of the results of the PYDI assessment tool.
  • Documents the progress participants make toward meeting their goals and works with the participant to develop new goals as needed.
  • Collaborates with community partners to address challenges, advocate on behalf of participants, and connect to services such as training.
  • Monitors and evaluates participants’ progress; provides guidance and coaching to help participants resolve issues and remain motivated to obtain and maintain employment.
  • Ensures participant information is timely entered into the appropriate form, database, or report and maintains confidentiality requirements.
  • Complies with the agency’s policy and procedures and Commission on Accreditation of Rehabilitation Facilities standards.
  • Performs other duties as assigned.

Qualifications

Required:

  • Bachelor's degree in Social Work, Counseling, Psychology, or related field.
  • Experience in case management, coaching, and service delivery.
  • Strong interpersonal and communication skills.
  • Ability to work effectively with diverse populations.
  • Proficiency in Microsoft Office Suite.

Preferred:

  • Master's degree in Social Work, Counseling, Psychology, or related field.
  • Experience working with individuals facing barriers to employment.
  • Knowledge of rehabilitation facilities standards and accreditation processes.
