Substance Use Counselor
Primary Job Function
The licensed counselor will be responsible for offering support and assistance to patients enrolled in Medication Assisted Treatment for opioid use disorder. Utilizing evidence-based practices and a trauma-informed approach, the counselor will help patients develop individualized coping techniques and symptom management skills that enable them to experience improved quality of life. Through collaboration with doctors, nurses and a multidisciplinary team, the counselor will play a vital role in evaluating patient progress and developing an individualized plan of care.
Essential Job Functions
- Provide individual and group counseling services; direct service at regularly scheduled intervals based on patient need, best practices and state regulations; coordinate all services throughout treatment.
- Conducts initial assessment and evaluation to determine eligibility for services.
- Complete Bio-psychosocial assessment.
- Establish individualized treatment plan with each patient and review as clinically indicated.
- Respond, as needed, to patient grievances and complaints.
- Develop aftercare and discharge plans.
- Other related duties as determined by supervisor.
Qualifications
- Must be an LISW, LPCC, LPC, AADC, or ADC.
- Satisfactory drug screen and criminal background check.
