About the Role
The Substance Use Care Coordinator assists the Substance Abuse Intensive Outpatient Program (SAIOP) Counselor with coordinating services across all levels of patient care. This role supports patients in addiction recovery through care coordination, organizing and hosting daily groups, and assisting the clinical lead in care delivery during groups.
Responsibilities
- Work with the Licensed Clinical Addictions Specialist (LCAS) to deliver SAIOP services, including administering urine drug screens (UDS), facilitating groups as needed, and providing coverage for the LCAS.
- Co-facilitate SAIOP groups weekly with the program counselor.
- Support patients in accessing care resources, including referrals to behavioral health programming, dental care, and supportive services such as Detox, Recovery Services, and Crisis Services.
- Remind patients of scheduled group therapy and other appointments, track SAIOP hours, and support referrals to step-down services.
- Adhere to AMH’s policies and procedures, maintaining an environment based on mutual respect and ethical communication with staff and patients.
- Maintain professional boundaries, consult with site supervisors on questions or concerns, and attend mandatory AMH and Behavioral Health Team meetings.
- Collaborate with the Community Resource Advocate to coordinate services that assist patients in increasing stability through community resources.
- Work with AMH teammates to provide wrap-around services that enhance patient care.
- Maintain CADC/CADC-I licensure and ensure timely, accurate, and patient-specific documentation in medical records for each encounter.
- Strictly adhere to all federal and state regulations related to healthcare, health information, patient care, and documentation.
Requirements
- Bachelor’s degree or equivalent.
- 2+ years of experience in the Mental Health and Substance Use Treatment field.
- Experience and knowledge working with individuals with substance use disorders and mental health conditions.
- Familiarity with group therapy and running therapy groups.
- Experience and knowledge of community resources and public benefits.
Skills
- Excellent interpersonal communication and community-building skills.
- Strong time management, organization, and ability to meet deadlines.
- Proficiency with technology, including documenting in Electronic Health Records (EHR).
- Ability to read, understand, and follow program guidelines and service materials.
- Willingness to work within clinic schedules and adapt to feedback for program adherence and safety.
Physical Requirements
- Must be able to read written and printed materials and use office equipment (computer, fax, copier).
- Must have sufficient hearing to communicate effectively with coworkers and others.
- Must be able to sit for the majority of the work shift.
- Must be able to stand intermittently and walk on hardwood, linoleum, and concrete floors.
- Requires manual and finger dexterity for office duties and responsibilities.
