About the role
A Substance Abuse Counselor provides individual counseling to highly complex patients, conducts clinical assessments, designs and updates treatment plans (including care coordination, referrals, and discharge planning), and manages an assigned caseload in compliance with clinical treatment requirements. You will collaborate with nurses and providers to deliver holistic treatment services and facilitate urine drug screening for patients.
Responsibilities
- Provides individual counseling to highly complex patients
- Conducts clinical assessments to appropriately assess patient’s treatment needs
- Designs and updates the patient’s treatment plan, including care coordination, referrals, and discharge planning
- Manages assigned caseload and compliance with clinical treatment requirements
- Collaborates with nurses and providers to provide holistic treatment services
- Facilitates urine drug screening for patients
Requirements
- General experience working with underserved populations in a clinical setting
- Lived addiction experience is a plus
- Qualified candidates will hold one or more of the following certifications/licenses:
- LMSW
- LCSW
- LPC
- LADAC
Schedule
- 37–40 hours per week
- 8:00 AM – 5:00 PM, Monday through Friday
