Responsibilities

• Affords assistance in the provision of case management for each patient assigned to his/her caseload, including contacting the referring agency as necessary.
• Completes required case management documentation within specified timelines.
• Conducts psychosocial assessments of all patients within designated time frames and documents findings.
• Documents patient behavior, interventions, and their effectiveness in the clinical record daily.
• Facilitates and documents group, family, and individual therapy sessions.
• Presents Master Treatment Plans to the Treatment Team and reviews them with the team.
• Attends Treatment Team meetings and treatment planning sessions to contribute to the patient’s treatment plan development.
• Researches, develops, and presents in-service training to staff.
• Trains milieu staff to conduct Skills Groups and co-leads these groups.
• Completes and charts the Initial Master Treatment Plan for each patient within required timeframes upon admission.
• Implements the Preliminary Treatment Plan within specified timeframes after the patient’s admission.

Qualifications

• Valid Driver's License.

Skills

• Demonstrates verbal and written communication skills.

Physical Requirements

• Extensive walking, standing, and sitting.
• Ability to lift over 50 lbs.
• Participation in physical restraint and seclusion.
• Completion of crisis intervention annually.
