Purpose of Position
Captures and implements stroke program.
Coordinates Performance Improvement in stroke care.
The incumbent collaborates with others to create systems of care which facilitate education, diagnosis, treatment and support for the patient, family and community.
Description
- Practices according to entity mandatory requirements.
- Practices according to department expectations.
- Practices with a customer service approach to all activities.
- Provides care as part of the interdisciplinary team.
- Utilizes internal and external resources.
- Provides effective patient/family teaching.
- Documents according to entity/department guidelines.
- Provides care according to best practice standards of care in nursing.
- Identifies signs and symptoms of neglect/abuse and reports as required by law.
- Cares for the terminally ill patient and family.
- Maintains appropriate licenses, certifications, registrations required to perform duties of their designated role/position.
- Maintains good communication with Brigham & Women’s Hospital in the Teleneurology program.
- Communicates as needed with hospital staff regarding deviations from protocols.
- Plans a minimum of 4 meetings a year with the Stroke Planning Committee and Co-Director to discuss changes in protocols, policies and procedures, outcomes, in the system.
- Communicates with the Co-Directors about issues with the Stroke program.
- Maintains good communication with the clinical areas where stroke patients are treated.
- Aids in community education regarding stroke recognition, diagnosis, and treatment.
- Must be able to work independently.
- Must be able to perform analysis of stroke patient’s charts in a timely manner.
- Must be able to get the information to stats dept. in a timely manner.
- Must be able to correlate outcomes with actual patient care.
- Keeps the program up to date for pediatric and pregnant stroke patients.
- Responsible for the overall budget of the Stroke Program.
- Performs other related duties as required.
- Challenges current working practices; identifies process improvement opportunities and presents recommendations and solutions to management.
- Engages and commits to the organization’s culture of continuous improvement by actively participating, supporting, and promoting CCHC Pillars of Excellence.
Qualifications
- Currently licensed as a Registered Nurse in the Commonwealth of Massachusetts.
- Baccalaureate Degree in Nursing required, Master’s Degree preferred.
- Graduate of an accredited School of Nursing; BSN Required.
- Master’s Degree preferred.
- Effective communication skills.
- Excellent presentation and facilitation skills.
- Demonstrated competence in quality data analysis and presentation.
- Minimum of 5 years of experience in Hospital with progressive experience in quality improvement preferred.
- Minimum of 5 years of experience in Quality Database and/or system management preferred.
Schedule Details
- 4 Day Work Week, Rotating Days, Monday-Friday, 8a-4p, Occasional Weekends & Occasional Holidays.
