Role Overview: The Stroke Program Coordinator is responsible for coordinating and implementing services and activities within the stroke program. They serve as a liaison among physicians, nursing staff, allied healthcare professionals, patients, and families to ensure seamless communication and care delivery. Key Responsibilities Facilitate stroke patient care across the continuum, ensuring adherence to clinical care paths.Collaborate with the multidisciplinary team to optimize patient outcomes.Coordinate activities and services related to the stroke program to maintain quality standards.Act as a point of contact for stakeholders involved in stroke care, promoting effective communication and coordination.
