Stroke Coordinator
Job Summary
The stroke coordinator is a health care professional who works in collaboration with the medical and hospital staff to coordinate multidisciplinary care to the neuroscience diagnosed patient. The coordinator provides clinical, professional expertise for patients, families, and staff members. Scope of practice includes providing organized, evidence-based care to the patient with neuroscience diagnosis focusing on health promotion and disease prevention. The coordinator also functions as an educator and coordinates neuroscience diagnosis care management to promote optimal patient outcomes. The coordinator will collect and analyze data related to stroke, assist with documentation, coding, clinical care and outcomes.
Qualifications
- Advanced Cardiac Life Support must be obtained within 30 days of employment start date
- Registered Nurse (RN)
- Associate Degree, or Bachelors Degree
- 3 years experience Required

The Stroke Coordinator attends and conducts Stroke Program meetings and is responsible for the evaluation of outcomes and policy.
