This is a remote opportunity within the Queens/LI and Brooklyn areas. The individual selected must reside in the Metro NY Region. Travel is required with a Monday–Friday schedule and no night or weekend shifts. 80% travel is required for multiple area assignments.

About the Role

Develops, implements, maintains, and evaluates an effective, data-driven quality assessment and performance improvement program with participation from the interdisciplinary team. Serves as a quality improvement champion by promoting Continuous Quality Improvement (CQI) principles to improve processes and patient outcomes at the facility and area levels. The role covers all treatment modalities (in-center, inpatient, home therapies) and involves collaboration with the interdisciplinary team to monitor data, prioritize improvement areas, and develop action plans.

This position offers the chance to serve as a quality improvement subject matter expert, influence clinic support, and contribute to career growth within the organization. Eligible for an annual bonus based on company and individual performance.

Responsibilities

  • Facilitates the implementation and integration of FKC Quality programs into facilities' standard operating procedures through collaboration with clinical, interdisciplinary, and operations teams.
  • Serves as a subject matter expert for clinical quality matters, collaborating with RNs and the Area Quality Team.
  • Identifies clinical quality improvement opportunities to meet CMS Conditions for Coverage and FMS quality program requirements.
  • Manages the tactical execution of Quality and clinical initiatives, interventions, and standardized education materials with facility RNs.
  • Provides nursing and clinical guidance to support facility RNs in achieving quality, patient satisfaction, teamwork, and employee satisfaction goals.
  • Collaborates with stakeholders (Clinical Quality leadership, Education, Regulatory, Compliance) to facilitate quality goals and patient safety improvements.
  • Mentors and trains staff to collect, trend, and analyze data to monitor clinical and operational processes, improving patient outcomes and satisfaction.
  • Utilizes evidence-based practices to address barriers to quality improvement, including promoting clinical algorithms, pathways, and policies.
  • Leverages tools, resources, and informatics technology to focus on targeted patient populations.
  • Applies knowledge of FMS policies, CMS Conditions of Coverage, and quality improvement principles to perform tasks.
  • Promotes collaboration between facilities to share successful processes and strategies.
  • Mentors facilities in identifying, testing, and adopting effective practices.
  • Provides guidance and expertise on quality-related policies, standards, and electronic applications under Clinical Quality leadership.
  • Performs desk reviews of facility Quality Assessment and Performance Improvement (QAPI) documentation and attends QAPI meetings.
  • Evaluates the effectiveness of facility QAPI programs and CQI activities.
  • Reviews completion of QAI activities, including adherence to the QAI calendar and tools.
  • Attends and participates in regional, area, facility, and team meetings, including quality team-building and staff development.
  • Collaborates with management to achieve effective interdisciplinary and clinic relationships.
  • Identifies risk areas and opportunities for improvement.
  • Assists with root cause analysis, action plan development, and evaluation.
  • Provides written or verbal recommendations to facility and area management.
  • Utilizes adult education principles to facilitate the implementation of quality standards and CQI practices.
  • Ensures outstanding customer service to all external and internal customers, including patients, staff, physicians, and payers.
  • Develops and maintains effective working relationships through timely communication.
  • Assists with various projects as assigned under Clinical Quality leadership.
  • Performs other related duties as assigned.

Requirements

  • BSN or Bachelor’s Degree in a healthcare-related field preferred, or an equivalent combination of education and experience.
  • Registered Nurse (RN) required.
  • Certification in Nephrology Nursing or quality preferred.
  • 3+ years of dialysis experience required.
  • 3+ years of management experience in a clinical leadership role.
  • Strong organizational, critical thinking, and customer service skills.
  • Demonstrated leadership competencies and adaptability to changing priorities.
  • Ability to work collaboratively with team members in quality improvement activities.
  • Strong verbal and written communication skills.
  • Ability to analyze and propose solutions for complex issues.

Physical Demands and Working Conditions

  • Day-to-day work includes desk and personal computer tasks, as well as interaction with facility staff and physicians.
  • Work environment is characteristic of a healthcare facility with controlled air temperature and moderate noise levels.
  • May be exposed to infectious and contagious diseases/materials.
  • Requires travel between assigned facilities and locations within the community (40-60%). Travel to regional, division, and corporate meetings may also be required.

Pay

Annual rate: $90,000.00 - $150,000.00 for the New York, NY location, depending on the candidate’s work location, qualifications, education, work experience, skills, and competencies.
