We are looking for an energetic, compassionate Licensed Practical Nurse (LPN) to join our team at VITALIA® Highland Heights in Willoughby Hills, OH. In this role, you will provide direct nursing care to residents in our Independent Living, Assisted Living, and Memory Care neighborhoods while supporting our philosophy of vibrant, maintenance-free living.

Responsibilities

• Provide direct nursing care to residents, including administering medications and treatments as prescribed
• Monitor residents' health status and report changes to the supervising RN or physician
• Assist with activities of daily living and promote resident independence
• Document care provided and maintain accurate resident records
• Communicate with residents, families, and the care team to ensure coordinated, compassionate care
• Respond to resident emergencies and provide appropriate interventions
• Ensure compliance with all state and federal regulations and community policies

Qualifications

• Current and valid LPN license in the state of Ohio
• Experience in assisted living, memory care, or a related long-term care setting preferred
• Compassionate, patient, and energetic approach to resident care
• Strong communication and interpersonal skills
• Ability to work effectively as part of a care team
• Commitment to maintaining resident dignity and promoting a vibrant lifestyle
