Step Down Navigator

About the role

The Step Down Navigator will assist in the design, plan, and implementation of programs designed to advance the marketing plans and services of the hospital.

Responsibilities

• Attend all treatment team meetings and advocate for step-downs
• Achieve 5% step-downs (Next Step Program) per week based on average daily census (e.g., average census of 80 requires 4 step-downs per week)
• Meet with all patients/guardians who have outpatient benefits according to their VOBs and provide an invitation to PHP
• Meet with Intake/Intake Director daily to receive outpatient referrals and follow up with every referral (minimum of 3 times, with documentation); review EMTALA for PHP/IOP referrals
• Attend daily flash meetings to report on step-down referrals; report on Business Development department in case of DBD/OP Director absence
• Communicate effectively with a variety of individuals and function calmly in situations requiring a high degree of sensitivity, tact, and diplomacy
• Provide prompt, efficient, and responsive service
• Update Outlook calendar daily with outline of daily tasks, prospecting, scheduled meetings, virtual meetings, and calls
• Complete the 8-hour shift required for the full-time position
• Verify communication by calling, text, or email
• Provide exceptional service as front person of GBHI
• Communicate politely with everyone coming to or leaving GBHI
• Complete outpatient utilization review and discharge planning duties as directed by the Director
• Complete other duties as assigned
