Starting pay $36.90 per hour, commensurate with experience. Sign-on bonus of $3,500 for external applicants, half paid on first check and half after 18 months (bonus amount prorated to FTE selected). Minimum of 16 hours per week with flexibility in days and times. Ability to work in the following areas: Columbus (43205, 43228, 43231), Grove City, Galloway, Granville, Hilliard, Powell, Reynoldsburg, and Upper Arlington.

About the Role

Provides direct, continuous skilled nursing care for acute, chronically ill, and terminally ill patients in the home setting as prescribed by NCH and community physicians, in compliance with regulatory bodies. Delivers developmentally appropriate care reflecting the highest standards of nursing practice to promote quality, continuity, and safe patient care. Practices professional nursing and patient advocacy in accordance with the ANA Code of Ethics for Nurses while maintaining a current Ohio Nursing License.

Responsibilities

  • Provides nursing care using a Family Centered approach.
  • Assesses individualized patient needs, communicates care needs, priorities, updates, and changes in condition to the assigned Home Health Nurse Supervisor or Manager and Staffing Specialists per policies and guidelines.
  • Utilizes Therapeutic Communication while monitoring the physical, psychological, and social needs of patients and families.
  • Engages in the workplace community by staying aware of changes, participating in data collection or auditing, and understanding Quality Improvement.
  • Collaborates to develop an individualized care plan using standards, protocols, and current nursing literature and research.
  • Implements and follows the individualized plan of care for each patient, stating patient outcomes, goals, and means by which these goals will be achieved.

Requirements

  • ADN required; BSN required within 5 years from date of hire.
  • Current Ohio RN license.
  • CPR certification.
  • One year of nursing experience.

Skills

  • Position-specific skills assessed at orientation and annual skill days.
  • RN I skills contingent upon experience and basic infusion care.
  • RN II skills contingent upon experience and complex infusion care.

Physical Requirements

  • Occasionally: Bend/twist, biohazard waste, blood and/or bodily fluids, chemicals/medications, climb stairs/ladder, communicable diseases and/or pathogens, depth perception, driving motor vehicles (work required), electricity, lifting/carrying 0-10 lbs, lifting/carrying 11-20 lbs, machinery, patient equipment, pushing/pulling 0-25 lbs, reaching above shoulder, repetitive hand/arm use, sitting, squat/kneel.
  • Frequently: Audible speech, color vision, hearing acuity, peripheral vision, standing, walking.
  • Continuously: Decision making, interpreting data, problem solving, seeing (far/near).
